I ordered one from Kiev Camera. I didn't really want to wait for shipping
from Europe. Hopefully it'll be okay.

I purchased one from Kiev Camera. While the tolerances seem
reasonably good, I'm not at all comforted by the fact that the index
dot is off by 1/8" or so. It also feels as if it would bind in cold
weather, but I'll have to wait a few months to find out if that's the
case.
